At around 3.10pm on Monday 8 May, an unknown man entered a chemist on Williams Street in Northbridge. The man submitted a prescription to the counter and waited outside until it was ready to collect. The chemist noted multiple spelling errors and inconsistencies, and contacted the prescribing doctor where the script was revealed as a fake. When confronted about the forgery, the man attempted to snatch the script back before fleeing the store.
Police have released CCTV images of a man who they believe may be able to assist with their investigation.
The man is described as: medium build, light skinned with short dark hair and facial hair. The man was wearing an orange hi-vis jacket over a black shirt, black shorts, beige work boots, a black cap and black sunglasses.
